I've spent 14+ years building software next to UX designers, and I kept watching the same thing happen on every team: the design is perfectly consistent in the file, and inconsistent everywhere else. Two greens that were supposed to be one green. A spacing scale that survives until the third sprint. A button that quietly becomes four buttons. Nobody is careless.
