How do you currently manage client feedback and approvals?

Most people I talk to are juggling feedback across WhatsApp,email, Notion, and voice notes all at once. By the time revisions are due,nobody knows what was final. Are you using a dedicated tool or just making it work with whatever the client prefers? Would love to hear what's actually working and what isn't.
