Facebook on the very large b2c side, Intercom on B2B and many other smaller companies who just experiment and iterate on product really deliberately.
This looks like an insanely awesome book. Full of actionable advice tailored around building out a process for building better products. Massive, but eminently readable.

it's very much dependent on your team. Start small and make them repetitive so they actually become habits. Incorporating more documentation is a good start. Finding ways to include important habits during weekly product meetings helps too.
