Agate Health - Disease History Summarization Engine

Hey Product Hunt! 👋 I’m Joe, co-founder and developer of Agate Health. Thrilled to share what we’ve been building! Agate Health started four months ago when I teamed up with my co-founder, Naciima. Both of us have personal histories of overcoming illness (I’m a two-time cancer survivor), and our moms were home-health nurses for years. This shared experience drove us to tackle challenges in chronic illness management. Through countless conversations with doctors and nurses, one pain point kept coming up: nurses spend hours copying and pasting patient data from EHRs, especially for complex cases. It’s grueling work. That’s why we built a disease-focused patient history report engine. Today, we’re launching the first piece: our Cardiovascular History Report, designed to streamline this process for cardiovascular disease--the leading cause of death in the U.S. This is just the beginning for us, and we’re incredibly proud of what our team has accomplished.
