How do you keep unplanned work from disappearing from your daily updates?

We’re launching Meridian tomorrow. We built it after repeatedly losing track of the debugging, unexpected fixes, and other unplanned work that never made it into our daily updates.

How do you capture that work today: manually, through standups, with AI tools, or some other way?
