I'd built a smaller version of this for myself a few years ago. It was just a simple way to turn incident notes and details into structured RCAs and incident reports. I used it after every major outage and it saved me a ton of time. No more blank-page syndrome or inconsistent documentation. It worked so well that I couldn’t imagine going back.
