Is full remote work causing silent misunderstandings on your team?

Noticed our dev speed was fast at first but lately we end up redoing work because everyone interpreted the spec slightly differently.
On zoom people just log off as soon as the meeting ends. You lose those quick 30-second casual chats where small confusions usually get cleared up.
Does anyone have a virtual setup that fixes this, or do you just deal with the extra rework?

i think unclear spec are a big part of it. two people can read the same thing and understand it differently.
I'm not in any way affiliated with this product, however I've been wondering, could something like https://www.gather.town/ make feel remote "less remote"?
I think it's a great idea and it could solve many of the "unhealthy" dynamics that full-remote can cause but it probably feels too niche to most of orgs/people.
