Here's the pattern I keep seeing: the customer's words live in a support tool, the revenue in billing, the work in a tracker, the code in version control four tools, none of them talking. So the two questions that matter most what should we build next? and did the last thing we shipped actually work? get answered by whoever argues loudest, not by the data.
I've become a little obsessed with the version of this that sounds simple but nobody's stack can answer cleanly. Mine is: "which of my paying customers are blocked on a feature we haven't shipped yet?" Every piece of that lives in a different tool, so answering it is a manual afternoon instead of a query.
