What makes the first work incident note easier to write?

I’m part of the Kirokune team at AMA CLASS, K.K. We’re introducing our iPhone app here, and I’d like feedback on the first-note workflow.

A blank note can be difficult to start. Our free web template separates three things:

• When the event happened and when you wrote the note

• Words you remember exactly and your own summary

• What you observed and how you felt

Which part would make a first note easier: clearer field labels, a shorter fictional example, or fewer fields? Feedback on the English wording is also welcome. Please use fictional examples, not private workplace conversations or personal details.

If you want to try the app, Kirokune organizes notes, recordings and photos in an iPhone timeline. No account is required. Unlimited records within one case are free; extra cases and PDF export are optional paid features.
