Care coordination today is fragmented β ACOs, virtual care teams, and delegated entities are often juggling disconnected tools for risk scoring, care plans, task routing, and reporting. We kept hearing the same problem from partners managing value-based contracts: their teams knew which patients needed help, but the workflow to act on that insight was scattered across spreadsheets, EHRs, and point solutions that didn't talk to each other.
That's what pushed us to build blueBriX Care Coordination Software β a single platform that unifies risk stratification, configurable care plans, team collaboration, and compliance reporting for ACOs, REACH entities, MA delegates, and virtual/hybrid care providers.
Along the way, our approach evolved from "build one more care management tool" to "build a layer that sits on top of whatever EHR or CRM you already use" β since so many of our early users didn't want to rip and replace Epic, Athena, or Salesforce. That EHR-agnostic, API-first approach became core to the product.
