What do you write down before a decision—not after?

I keep noticing the same thing in my own work: once the result arrives, my memory becomes a better lawyer than historian.

I'm trying a simple rule: before an important decision, write down the probability, the reason that could be wrong, and the date you'll look again.

I'm curious about other people's real habits, not ideal ones. When a decision has delayed feedback—a hire, launch, price change, or investment—what do you actually preserve beforehand? And what feels like too much ceremony?
