Organization's Knowledge retention problem

When someone leaves your organization, 70% of knowledge goes with them. What kind of knowledge codification or digitization tools or processes you have seen working effectively? Or this is a big pain point for you?

Hey Joe. Yes you hit something here. Some thoughts: 1. We usually aim to add our knowledge in a wiki on a regular basis as part of our team norms - this is a good start. 2. Another thing I did is to try to coach the teams I work with to share knowledge and build new skills. So if you see that someone is good in say soft skills - try to find a was to transfer hers knowledge to the team. What experience do you have?
