Hey PH! 👋 I’m Faraz Ihsan, founder of Patient Diary.
If you’ve ever seen a doctor juggling paper files, random Excel sheets, and a dozen WhatsApp notes just to keep track of their patients, you’ll understand why I built this. In my network, doctors were spending way too much time organizing data instead of focusing on care. So I rolled up my sleeves and built a tool that feels like a natural extension of how they already work not another bulky EMR.
What is Patient Diary?

Why I actually built this
I built Patient Diary because I saw friends and colleagues and doctors who are already overworked writing data on paper, then re‑typing it into Excel, then emailing files around, just to answer basic questions. That’s not how medicine should feel. Patient Diary is my way of shrinking that chaos and giving them a simple, human‑friendly way to:
