How is it different from an ultrasonic toothbrush?

This is pretty much the wrong way to brush your teeth.
The reason brush heads rotate horizontally is to "jiggle" out bacteria and plaque from under the gum line before it forms calculus (tartar).
All this is doing is cleaning the surface of the tooth, while that's important also, it's more important to remove the food particles and bacteria at and just below the gumline. It also doesn't seem to clean interproximally either- the disclosant left behind is not shown in sextant 2 (the top teeth- canine to canine) in the promo video- the brush is hiding the results.
When a hygienist cleans your teeth, they use special instruments to remove the plaque from below the gums. The "brush" they use at the very end isn't a brush - it's a prophy cup with prophy paste that is using a speed and grit that is not available OTC and that is what polishes your enamel.
I also find it interesting that no one in the video is actually cleaning their teeth. When I use my Oral B - it makes a bit of a mess; and that's with oscillating brush heads. Could you imagine the mess this will make? Flicking water and tooth paste out from your mouth onto a mirror?
Also, you brush for 2 minutes, twice a day. It's not even just with a manual brush- the time is to ensure you are spending at least 30 seconds in each quarter of the mouth getting every surface- inside and out. Which brings up another point, this seems to not adapt to the tooth surface very well at all, which is also important in proper oral home care.
Plus, all those acronyms don't mean anything unless you have ADA approval, now, usually powered toothbrushes don't have an ADA seal, but that's because they do not differ from previous, proven models. From the ADA's rules on its product acceptance requirements:
B. Powered toothbrushes
i. Clinical efficacy studies will normally not be required for powered
toothbrushes if they meet the above requirements for safety and do not differ
significantly in design or function from previously Accepted powered
toothbrushes.
ii. If in the opinion of the Council the toothbrush does differ significantly in
design or function from previously Accepted powered toothbrushes then a 30
day clinical trial of at least 25 subjects per group demonstrating that the
product can be employed under unsupervised conditions by the average
layman to provide a 15% statistically significant reduction versus baseline in
gingivitis and a statistically significant reduction in plaque is required.
I would interpret this video's explanation of the toothbrush head to be nothing like anything before and would like to see a study and/ or ADA approval before I'm convinced it actually doesn't damage enamel or even removes the plaque like it claims, not even mentioning to make sure the level of abrasiveness is safe to use on enamel.

It is well-known that, in order to maintain a proper oral hygiene, the outer surfaces of teeth should be cleaned by brushing them exclusively from the gum margins towards the dental crown. As a matter of fact, it is extremely important to remove the bacterial plaque from the surfaces at and below the free gingival groove, and from the tooth-gum interfaces, where the said plaque naturally tends to hide.
Cleaning the teeth with a proper brushing movement is particularly difficult, when employing a conventional toothbrush, especially at those places in the oral cavity where there is no easy access.
The reversible motor actuated by one switch, and may drive the brushing head the bi-directions. Thus, the user may choose the correct direction of operation in order to obtain a brushing movement of the bristles always from the gingival groove area towards the crown area.
The effectiveness of the rotary toothbrush according to the invention has been investigated by means of several tests , some of which are reported below. A first subject examined did not show any particular pathology but an unpleasant cold feeling at the neck of the 5th lower right tooth. The free gingival margin was not inflamed, and the periodontal probe did not penetrate more than 1.5 mm. Therefore, the sensitivity at the collet was to ascribe to an improper use of the conventional toothbrush in view of the remote location of the concerned tooth. After 6 days of use, 2 times a day, of the toothbrush , the sensitivity had completely disappeared, and after 12 days the gingival margin had migrated towards the - crown, thus covering that zone of the collet which was responsible for the said sensitivity.
Another patient, aged 55, had problems with the gum margin in all sections of his mouth, owing to an incorrect oral hygiene. The interdental papillae were hypertrophic, and showed edema and bleeding; the periodontal probe penetrated for 3-4 mm at all locations and the central lower right incisive showed a V-shaped gingival recession of about 4 mm in the apical direction and 3.5 mm in the mesio-distal direction. After using one week of brushing the gum margin was less inflated and less reddened, and the bacterial plaque was completely absent, as shown by employing the plaque detecting solution. After 7 days the normal conditions were completely restored, and the periodontal probe penetrated for not more than 2.5 mm. After 15 days of treatment the V-shaped retraction of the gum margin was reduced to one half, both in the mesial and in the apical direction; the collet zone was sensitive only to strong thermal shocks.
The above test shows that when the bacterial plaque is prevented from resting in the gingival groove there not only is a reconstitution of the tooth-gum interfacial epithelium but also of the connective collagen fibres, which thus reform the physiological barrier which blocks the access to toxic substances to the zone below the gum margins.
