I think SurveyMonkey is one of the best tools in this regard.
Since we operate out of Discord, we use Discord. Not all users in our server, so we polls on our blogs and ask to send feedback to email- although I am wanting to create a widget or function on the website where they can click to input feedback, suggestion, advice, or report bugs.
