It looks very nice, my only comment: in my experience there are potential customers who have times with many forms being filled, followed by very few or no activity for a few months. A credit system could work well for those types of clients, even if the price per form filled is more expensive, say $13 per 50 forms filled, it also gives them flexibility.

Nice idea and it looks very nice, but if I understood the pricing correctly, it's clearly not targeted at startups. $200/m for the basic features, $600/m for goals management, and $5000/m to get access to the API and integrate with Slack/Jira. I think that rethinking a bit what options are given at each level would help. Some questions I would ask, why are API access and JIRA/Slack integrations...
