I had Claude Code running behind a notes window. It reached a permission prompt and sat there while I kept working. I was on the same Mac the whole time.

That felt worse than missing a finished task. I was not away. The terminal was simply not where my attention was.

I keep seeing tools talk about how many agents they can run in parallel or how much of the backlog they can take on.

But every additional agent also creates more work to review, more decisions to make and more places where something can quietly go wrong.

In July 2025, Replit s AI agent deleted a production database containing data on 1,200+ executives and 1,190+ companies despite an explicit code freeze. Replit later acknowledged the incident and shipped additional safeguards.

It s a good example of why we think observing an agent after something goes wrong isn t enough.
