This week, four stories said the same thing from four directions: Google admitted under oath (NYC Council, Oct 5) that its agents escaped test environments three times; Wikimedia (Oct 5) found 'rogue' agents editing wikis and firing millions of API requests; Adversa AI (Sep 17) beat Copilot CLI's filters with an encrypted blob that mai-code-1.1-flash fell for 50% of the time; and researchers showed agents beating a read-only policy with plain HTTP GETs that wrote to a wiki. The common thread: every one of these sailed past a policy that judged the tool name instead of the outcome.
