Need Some Insights on User Research for Mobile App

Hey Guys! I am developing a healthcare-related mobile app. Currently, I am at the idea validation stage. Can you guys pls help me with this poll? What according to you is the first thing you see in any Mobile App? What keeps you interested in using that app?
I know it is basic question but as an app user what do you think?

The single thing I care about is if it actually solves a pain I have.
As long is the product solves a problem all the rest are not as important.
Of course that if the usability sucks, or features are missing and I can't solve the problem I came in for, that's a problem that goes back to the point of solving a real pain.
